Here is one example: The initial production run of a computer supply had a high failure rate. By doing extensive testing I determined the power supply was failing while being powered down. Design changes were made to the FET gate drive circuit and the return rate decreased by 95%. Also, the output filter and voltage feedback were changed to decrease “coil whine”.
Finding what causes failures is like detective work or solving a puzzle. Early on in my career I found that by adding a 5-cent capacitor the failure rate on an uninterruptible power system product was reduced by 90%. It took a week of testing to find the cause and solution.
