suggestions?
Disable Windows core isolation memory integrity. Exit ThrottleStop and delete the ThrottleStop.INI configuration file. Reboot and post an updated FIVR screenshot. Your screenshot shows that you have Windows Virtualization Based Security enabled. VBS is not compatible with undervolting. Read the links in my signature if you are still having problems.

When ThrottleStop is working correctly, you should not see a column of 0.3799 values in the FIVR monitoring table. That is a big red flag.

Having issues
What sort of stability testing did you do to prove that your undervolt settings are stable? Does the TS Bench 960M test report any errors? That is a bare minimum stability test. I would also run some 3D Mark tests. Check the ThrottleStop Log File box when testing so you have a record of your CPU performance and temperatures.

Of the three screenshots you posted, the first two are from Oct 25th when VBS was enabled. The third screenshot from Oct 31 looks OK.

If games are crashing, your computer is not stable. Reduce the undervolt. See if you have any problems with the undervolt at -50 mV for the core and the cache. If you are still not stable then set the offset undervolt back to 0 mV.

Edit - I would set IccMax to the max, 255.75, for both the core and the cache. Post a TPL screenshot.
