RX 470 4G Mining - Getting up and running / GPU identification help

Hello,

I recently acquired what I think is a Mining edition RX 470 4 gb GPU. I noted one single HDMI port under the metal front plate as shown in the last pic. It appears to run and post in the device manager and once pixel patched, is recognized just fine by Windows. Assuming they have a modded mining Bios of some kind maybe. The issue I am having is when I go to mine anything with it, I cannot get it to benchmark on any algo and when I do manage to pass a benchmark to mine, it won't even try to start mining. I figured this thing should rip up some Raven. I have tried every version of adrenalin drivers I found from 16-18 to the latest 24. I even tried the old blockchain drivers I had on a CD. I am wondering if I need to flash the Bios to get this thing working/mining, maybe swap to a different driver I have not tried?! I have a fair amount of experience with computers in general, GPUs and mining, but this thing is stumping me and I am kind of lost. I cannot manage to even identify exactly what it is, who manufactured it, or even find an original picture of one! The GPU-Z says its a Sapphire when I click lookup, but the form factor and board look very different. All the benchmarking and testing I have done says everything works just fine in the card. Please see below pics of GPU-Z screen and tag. I appreciate any help or direction!

Someone is offering a similar card on ebay (https://www.ebay.com/itm/186410828597).
It doesn't look like a sapphire card. Like requested by eidairaman1, please remove the cooler and make clear pictures of the PCB.

Please see attached pics. Really appreciate the help!

215-0876406 is a mining chip.
H5GQ4H24AJR is used on 4GB cards.

One of these bioses might work:

Please follow the mini flash guide below. Note: only use AMDVBFlash 2.93.

For RX 500 Series (Polaris) and Older:

Get amdvbflash 2.93

https://www.techpowerup.com/download/ati-atiflash/

Then look for Version 4.68, underneath 4.68 you will see a link to show older version, click that and you should see version 2.93

Follow these steps carefully:

1. Open your "C:" drive in Windows, create a new folder, call it atiflash.

2. Extract the contents of atiflash_293.zip to the "C:\atiflash" folder

3. Find the bios file you downloaded and rename the bios file to 5 digits with .rom on end Example RX 580, change file to rx580.rom (r****.rom)

4. Move r****.rom to the "C:\atiflash" folder

5. Open the start menu, type cmd, when cmd.exe appears right click it and hit "Run as administrator"

6. At the command prompt type in cd\ and press enter until only C:\> is on the screen.

7. Type in cd/atiflash and press enter, you should see C:\atiflash>

8. Type in amdvbflash.exe -i to identify the gpu you want to flash with a 0, 1, or 2)

9. At the C:\atiflash> prompt type in
amdvbflash.exe -unlockrom 0
(there is a space between the e and -unlockrom and 0)(0 is the number zero)
press enter and you will be prompted that the "ROM Unlocked" (or 1, 2)

10. At the C:\atiflash> prompt type in
amdvbflash.exe -f -p 0 r****.rom
(there is a space between the .exe and -f, and a space between -f and -p and 0).
(Or 1, 2)

Press Enter

Follow directions on screen and if flash is successful exit the command prompt and restart, windows should show up on screen.
