Doesn't look safe to use with a graphics card, even if the mobo is horizontal.

The product's page demonstrates putting - with the help of a second adapter - 6 NVMe drives on one PCIe X16 slot. So it could be a nice option for those having a lot of NVMe drives, on systems that aren't used for heavy gaming.
In the product's page they also demonstrate an M.2 to SATA adapter, so someone could also get extra SATA ports that way. I mean someone could buy a microATX board and end up having a number of NVMe and SATA storage drives connected on that board, if the board supports splitting the port to 8x+4x+4x.
There are also adapters that give typical PCIe x4 slots out of an M.2 slot, meaning with two M.2 to PCIe slot adapters and two PCIe x4 extender cables, someone could mount extra PCIe cards. With many modern motherboards limiting PCIe slots to 2 or 3, this could be a solution.
But it could also be used with a mid range (at least) graphics card with the help of a good quality PCIe X16 extender, where the graphics card could be mounted vertically, if the PC case used has vertical expansion slots.
While not the most secure way to expand the available options of a motherboard, the most secure being having those options by the motherboard itself, there are cases where something like this could help.
