Games not smooth, despite having 240hz screen/240 fps, good temps etc.

Should be capped at around 3 fps below monitor refresh rate to reduce input lag IIRC. Your issue may be different but may as well start there and show people you're willing to take at least some advice.

bro, I said that I've tried to cap 237, no changes
seems way better, but I think I can make it even better, only did 2 changes, one in Nvidia the second in Device control panel

How can I change it so I don‘t have this problem anymore? I turned off PowerThrottlingOff in regedit since I had it there ( my pc is tweaked) and it helped a little bit the latency only goes to 800 and not always. Is there a way to get rid of it forever?I'm guessing, power management settings.
