Well I'm not quite sure what mxm-sig spec says, but most power hungry mxm cards are 200W.
The higher-power cards are oversized to fit in more VRMs, and then the cooling is similarly increased in capacity on the laptops they go into. Hopefully these'll get properly standardized into the spec soon and Dell/Alienware goes MXM again (they switched to soldered with the Skylake refresh).
