What programs do you have in the Start Menu? as I can see that your GPU clock is also on. Do you have Icue or some other Hardware log running. This might seem crazy but it could also be your Cable. Not all Cables are made equal.My question exactly, and no I don't mine, no idea how to.
I got my power draw down to "normal" in all resolutions at 60 Hz without AMD noise suppression (I had that active earlier) , but still high 90-100w in 120 Hz. Tried a different HDMI 2.1 cable and changing the power plan, but same results. Have anybody tried 4k 120Hz with HDMI 2.1 vs DP? I only have one monitor, and it that doesn't have DP.So after checking a few things, not really one to one results because I was running YouTube to check things and then changed PCI express power settings.

First interesting tidbit, adrenaline seems to increase idle power usage; my guess is HW acceleration used for their monitoring software and graph plotting. Use only GPUZ or maybe Hwinfo - instantly dropped 15-20w depending on monitor number.
Second thing, and I haven’t tested impact on gaming performance, try enabling maximum power savings on the pcie setting in the power plan; this may have other effects I’m unaware of at the time but it drastically reduces idle/youtube power consumption by 20-40w.
Third, either my gpu/AMDs driver hates my 1080p monitor, or the hdmi cable I’m using for it is terrible. Side note my 48” c2 is hooked up via an optical hdmi cable.

I used DDU, and gpuz shows 90w. It's only high power use when I use 120Hz on my displayDid you simply upgrade from 23.5.2 to 23.7.1? I used DDU before installing. Might help. Also don’t use Arenalin for power monitoring, try gpuz or Hwinfo and see if it’s any different for idle measurements.

I find I have no problems as long as I use DDU to uninstall drivers in safe mode with no networking. I make sure to download drivers from AMD before I uninstall and after running DDU, I make sure no internet, reboot and install drivers and only then turn back on network. I have had issues several times doing a normal install via the AMD control panel and all were fixed by doing it the slightly more tedious way.Two months for this crap.
This driver causes crashing in Last of Us and Metro Exodus Enhanced. The control panel size also doesn't save.

interestingly, I never used ddu. it was never necessary. I keep on updating all the drivers. I even transitioned through different amd gpus without ddu. rx 6900 xt - rx 6600 - rx 7900 xt.I always use DDU as well (In Safe Mode). Same issues.
I re-installed 23.5.2 and everything is fine again so it's definitely the drivers.

The problem is, it's random, someone can run 4k/120hz and have the high idel usage and another person with a different setup but same 4k/120 have the lower idle, I run 1080p165hz and have had high idle usage, was using a may 22 driver for months until it got fixed, not had the issue since then but it is persisting for some users regardless, that said AMD are still releasing fixes for it with several different drivers so no it's still an issue for some while for other it has actually been fixedJust gonna weigh in here on the VRAM usage. I also have it. It’s down to high refresh rate Monitors.
As @DanielAron discovered lowering his refresh rate to 60hz.
I don’t like anymore than anyone else as a single high refresh monitor user(144hz)
I believe even Windows itself is giving a warning somewhere about higher refresh may consume more power(don‘t quote me on that but the warning IS somewhere
I’m not 100% sure on these publics if Factory Install has been reenabled again yet but it is a WAY better option than using DDU now. I’m on betas where its still available but if anyone could give me any confirmation on it(I usually install the Publics just to check against my Beta Branch anyway at some point)

The funniest thing is while it running the ram at full speed on my desktop 95% of the time if say open Discord it drops to like 900 ¯\_(ツ)_/¯
Now granted I am on a different driver than you this issue is as you say persisted in certain configurations for quite some time.
Like for example right now I dropped my refresh from 144 to 60 my VRAM usage dropped to 900/30W. I bump it back up to 144 it jumps back up to 1340/36W
