I wonder why 1142A the only one that has an internal header. They (or someone) needs to make a card with a mix of external USB3/eSATA6 with internal SATA and a USB header. There's so many old computers out there that could use a kick in the ass from SATA6 + USB3 board.

My ideal board would have two internal SATA6 (must support hotplug), two external USB3, and one USB3 header.

What about the ASUS U3S6 card? It was discontinued a while ago but it did solve this problem. (It didn't have internal USB 3 headers though)

The only way now is the two card solution to upgrading older PCs, but it's a lot more common for a PC to have two spare PCIe x1 slots than one spare PCIe x4 slot. You can get a VIA VL810 USB 3 PCIe x1 controller card with internal header and 2 external ports for $20 and a SATA 3 PCIe x1 controller card with internal ports for $12.
